Applicant's tor the Cystic Fibrosis Patient Assistance Program (CFPAF) must meet the following eligibility criteria prior to enrollment:
Applicant must be diagnosed with cystic fibrosis, with pulmonary complications
Applicant's household income must not exceed 400% of the Federal Poverty Level (or 500% if there is more than one CF patient in the household)
Applicant must be using an FDA approved medication for inhalation or a paired drug delivery device for the on-label, nebulized treatment of cystic fibrosis pulmonary disease.
Applicant must be a permanent residence of the United States.
