Do I Qualify

Eligibility Criteria:

Applicant's tor the Cystic Fibrosis Patient Assistance Program (CFPAF) must meet the following eligibility criteria prior to enrollment:

• Applicant must be diagnosed with cystic fibrosis
• Applicant's household income must not exceed 400% of the Federal Poverty Level (or 500% if there is more than one CF patient in the household)
• Applicant must be using an FDA approved medication or paired drug delivery device for the nebulized treatment of cystic fibrosis related pulmonary disease, or an FDA approved medication for the treatment of pancreatic insufficiency related to cystic fibrosis.
• Applicant must be a permanent resident of the United States.
