The Cystic Fibrosis Patient Assistance Foundation (CFPAF) is a non-profit organization that helps patients and their family members living with cystic fibrosis afford the medications and devices they need to manage their disease. We have designed the CFPAF program specifically to meet the needs of the cystic fibrosis community - making us unique among national patient assistance programs.

If you would like to apply for co-pay assistance with CFPAF, please click Do I Qualify? to determine whether or not you qualify for our patient assistance program, and to download a writable and printable application.

Once your application has been submitted, you can expect to receive a follow up telephone call within 5-7 business days after the application has been received by our office to discuss your program eligibility.
