If you are a health care provider and would like to help a patient apply for assistance through the Cystic Fibrosis Patient Assistance Foundation (CFPAF), please visit How To Apply. If your patient appears to be eligible for the CFPAF, then you may download an application from the website or you may request an application by calling us at (888) 315-4154.
Once you and your patient have completed the application, please submit it and the required documentation to our secure fax at (877) 868-5952. You can also mail the application and accompanying documentation to us at the following address:
Applicants can expect to receive a follow-up telephone call from one of our Case Managers within 5-7 business days after their application has been received. Our Case manager will review the application with the applicant to make sure that it is complete and that we have all of the required information.
